If abnormalities are found in basic parameters, such as impaired sperm motility, morphology, or concentration, or if fertility issues persist despite normal semen test results, additional diagnostic investigations are recommended.

The semen analysis packages provide comprehensive and rapid semen diagnostics, offering detailed information on sperm structure, maturity, fertilization potential, as well as genetic and immunological abnormalities.

We recommend semen testing:

  • To broaden the diagnosis of male infertility;
  • To diagnose consecutive failed natural attempts and assisted reproduction procedures;
  • Tn case of recurrent miscarriages;
  • In case of infertility of unknown cause (idiopathic infertility);
  • Prior to assisted reproduction procedures in order to select an appropriate sperm selection method and treatment method.

The following factors should be considered before proceeding with semen testing:

  • Sexual abstinence

One of the key factors that affect the reliability of semen analysis is sexual abstinence, which should be from two to seven days. Both shorter and longer periods of sexual abstinence can affect parameters such as concentration, motility and sperm morphology. In the case of a repeated examination, the patient should maintain the same period of sexual abstinence.

  • Stimulants and physical activity

Before semen testing, you should maintain a minimum of two days of alcohol abstinence and also limit excessive physical exertion for at least a day before the test.

No additional studies are required other than observing the standard semen preparation plan.

To perform the tests included in the basic semen analysis package, fresh semen must contain more than 1 million spermatozoa per milliliter. The basic package is not available for patients with known azoospermia or severe oligospermia.

In the extended package, a minimum of 30 million motile spermatozoa (moving in a progressive motion) must be present in fresh semen to perform the tests it contains. Some tests contained in the extended package are not possible to perform in patients with known azoospermia and severe oligo-, astheno-, and necrospermia.

Semen testing should not be carried out during antibiotic therapy, in which case you should wait a minimum of 14 days after the end of the therapy. The result is also negatively affected by an elevated body temperature – in this case, it is recommended to perform the test 2.5 to 3 months after its onset.

Course of testing

Stage 1

Registration

The medical registrar labels and dispenses a sterile container to the patient and then escorts the patient to the sperm donation room, where the patient is instructed on the test and procedure.

Stage 2

Semen donation

Material for analysis should be donated by masturbation directly into the container, after urinating and washing the genitals beforehand. You are required to donate all the material. If for some reason part of the semen is passed outside the container, the laboratory staff must be informed. The material must not originate from a condom or intermittent intercourse.

Stage 3

Macroscopic analysis

The sample is sent to the laboratory. In the first instance, macroscopic parameters are assessed; sample liquefaction time, volume, pH and viscosity are measured.

Stage 4

Semen testing package I and II – basic

In basic package I, a manual semen profile is conducted. After macroscopic semen analysis, sperm concentration, motility, and morphology (also known as structure) are assessed. The presence of round cells (including leukocytes) is also assayed. Basic package II features computer-assisted semen analysis SCA. In addition to the macroscopic analysis of the ejaculate, appropriate computer software allows for detailed analysis of motility and accurate analysis of sperm morphology – based on precise measurements of the length, width of the head and insertion, their shape, the angle of their junction and the size of the acrosome. The concentration of round cells (including leukocytes) is also measured. Next, both packages include the SCD Test (nuclear chromatin dispersion test, a DNA fragmentation test), which is done with certified HALOtech tests, by denaturing and staining the sperm chromatin. During the test, intact DNA strands expand to form halos around the sperm heads, creating what is known as the halo effect. For spermatozoa with damaged chromatin, the halo effect is weak or absent.

Stage 5

Semen testing package III – extended

In addition to the tests included in basic package II (computer-assisted semen analysis SCA and DNA fragmentation test), the extended package includes the HBA test. The HBA test is done by applying a sample of fresh semen to a hyaluronic acid-coated slide. Microscopic analysis of at least 100 motile spermatozoa is done after several minutes. To confirm a normal test result, a minimum of 80% of sperm must be bound to hyaluronic acid.

Stage 6

Semen testing package IV – full

In addition to the tests included in extended package III (computer-assisted semen analysis SCA, DNA fragmentation test, HBA test), the full package also includes the MiOXSYS test and IgG and IgA class MAR tests. The MiOXSYS test involves applying the semen sample to a sensor placed in the analyser, which measures the oxidation-reduction potential. The MAR test is performed by adding latex beads coated with IgG and IgA antibodies, along with anti-IgG and anti-IgA antibodies, to a semen sample, followed by microscopic analysis of at least 100 motile spermatozoa. If IgG antibodies are present on the spermatozoa, sperm fuse with globules coated with the antibody. The IgA class MAR test is performed similarly.

Stage 7

Test results

The test results are sent to the patient’s email address via an encoded .pdf file, signed by the diagnostician with an electronic qualified signature. We usually have the test result available the next day, and no later than within 3 business days.

We have two semen donation rooms in our clinic. These are located in a secluded part of the corridor, away from where other patients are waiting for their appointments. The room is equipped with a washbasin, a comfortable armchair and a TV with videos to help you donate sperm. The room is lockable from the inside and there is a tag on the door to advise whether the room is free or occupied. The room is communicated with the laboratory by an administration window that is locked from the inside; you leave your material for testing in this window.

In exceptional cases, the material can be delivered to the clinic from home in a container labelled with the patient’s full name and hour of material donation. You must then bring the sample in person to the laboratory no later than 30 minutes after ejaculation. During transport to the laboratory, the semen should not be subjected to shocks or temperature fluctuations (optimum transport temperature is 20-37°C). The transport temperature is most easily controlled by transporting the container in contact with the body, e.g. in a shirt or jacket pocket.
